The deep transverse metacarpal ligament (DTML) is a structure that extends between the head of second to fifth maintain stability arch. Injuries this due blunt trauma produce DTML injury and present with pain, decreased grip strength, deflection diastasis affected fingers. Several surgical techniques for its reconstruction have been described. However, we evaluated evolution two patients who presented diagnosed in acute treated conservatively. Immobilization forearm plaster intrinsic plus position syndactyly adjacent finger was performed 4 weeks. At one year follow-up, they no pain (VAS 0/10) showed hand function comparable mobility contralateral We consider face lesions period, diagnosis through clinical radiological evaluation, as well nonsurgical treatment, are usually sufficient.
